Heart Failure Transportation

My name is Thomas and I have stage 2 HF with low Ejection Fraction.I have a Pacemaker implant and a lot of Doctor appointments to get to but our trucks Transmission is going out.I am Currently on Social security and cannot afford to get the Transmission replaced. Looking at cost of a replacement and to have it installed is very expensive.Its a 2011 Ford F150... Any help towards my goal would be greatly appreciated.
